Working closely with our leading nonprofit partner, BAYADA Home Health Care, Integrity offers high-quality care to Mercy Health System patients, as well as others in our community who are either:

  • Managing a chronic condition

  • Recovering from a recent surgery, illness, or hospitalization

  • In need of help or companionship to continue living safely at home

  • Discontinuing curative treatment for a life-limiting diagnosis in favor of therapeutic and palliative (comfort) care focused on quality of life and well-being

How home health care works

We make it simple and easy to get the coordinated care you need. Home health care services, which must be ordered by your doctor, may include nursing, and physical, occupational, and speech/language therapies to help you recover or manage a chronic condition in the comfort of your own home. How hospice care works

For those eligible for hospice, Integrity delivers expert medical care including symptom and pain management, as well as social, emotional, and spiritual support through the end-of-life process. We are committed to providing personalized care, so patients and families feel empowered and supported during life's final journey and beyond, through loved ones' bereavement process. Integrity's interdisciplinary team of hospice experts has one goal in mind: to focus on people and their meaningful time together, enabling patients to live in dignity and comfort in a private residence or congregate living facility—wherever they call home. Safe, supervised medical care—at home

Timely start of care
We will be there when you need us to begin services, usually within 24 hours of your call.

Trained and compassionate caregivers who love what they do
Depending on the service you receive, your team of visiting caregivers may include nurses, therapists, medical social workers, and other skilled medical professionals who specialize in the care you need.

Clinical supervision
All caregivers are screened, licensed, experienced, and matched to meet your unique needs, and each patient's care is supervised by a Registered Nurse (RN) Clinical Manager.

24/7 availability
If you have a question or are experiencing a crisis, you can reach us anytime, day or night.

Family support
For those receiving home health services, we provide education to help you and your family understand your medication management and treatment plan with confidence and assist you with follow-up steps, such as making appointments or providing counseling services as needed, until you have met the goals of your personalized care plan.

For those receiving hospice care, our support services continue after a loved one's passing with ongoing bereavement counseling for family members.

Paying for services

Medicare is the primary payment source for both home health and hospice services. Medicaid and private insurance are also accepted if you do not have Medicare benefits. Most patients will pay nothing out of pocket for the cost of care.
